    Your <12 detected is approximately the same as <25 undetected in the US because the ultimate test sensitivity is roughly 1/2 the quantification level.

    In your favour - ?female, 1b, low viral load, ?treatment naive

    Not in your favour borderline cirrhosis with 12.5kPa on fibroscan

    In and of itself a <12 detected at 4 weeks would not worry me that much. The chances are you will clear with 12 weeks.

    It has/will always be the case that longer treatment gives slightly better results but we are talking and extra 100% cost chasing the last ~5% cure.

    So in a nutshell you could argue either way. With your 12.5kPa we certainly want to cure you sooner, rather than later. On that note - surely you should have qualified for NHS Harvoni?

    Hello countless,

    Fortunately the medications don’t really require much monitoring.

    Liver functions at 4 weeks should look good and provide reassurance, same at EOT and SVR4 and then just a viral load at SVR12.

    The monitoring has essentially zero impact on treatment outcome, and is simply not done in many countries using DAAs.

    Is “reverse” causal relation possible? For example, people having healthier liver, without cirrhosis – may just better tolerate coffee (and, consequently, may have statistically higher coffee consumption).

    This is indeed possible. It is the difference between observation and clinical trial. The clinical trial would be to have a large group assigned to drink coffee and another large group assigned to abstain. Because of the timelines involved – say a decade – this is unlikely to ever happen.

    My GP has sent me and consultant a letter stating she is not going to be involved in monitoring as it is beyond her expertise and responsibility of a consultant

    Hi Tommy,

    Thanks for posting this.

    Whatever your point of view, the problem for us – as in we, the people – is that even combining our fund (via tax or insurance) Big Pharma has pushed it so far we have reached to point of not being able to pay.

    I expect fiscal reality will see the pendulum swing back to somewhere better.

    Now eventually there may be an exception but in my experience normal bloods at SVR12 and UND go hand in hand.

    I should really say “In my series”. Here is secret doctor code:

    I am familiar with the procedure = “I’ve seen it done”
    In my experience = “I’ve done one”
    In my series = “I’ve done 2 or more”

    I have now seen over 1000 patients achieve SVR and not one has had normal LFTs and a viral load.

    Don’t be the first!
