    Hi Archer,

    Welcome back. :) As always you raise a number of important points most of which are far outside my experience but there are a couple of comments I would make.

    For your concerns about your recent symptoms and the impact of your diet, I’m also at 22 and a bit weeks Tx and also have experienced some increases in aches, twinges and itching recently. I suspect it is probably just the length of time on the meds, either physically or emotionally, rather than the virus regrouping because as you say being on treatment effectively knocks any viral activity out. From a dietary perspective even if sometimes we haven’t been fully adherent up till now (goodness I like pizza now that I can digest it! ;))* I suspect that with undetectable viral activity in our blood, the virus is unable to manipulate any excess sugar to allow massive replication. I think the important thing now is to get our dietary sugar under control prior to finishing treatment and then strictly maintain it until we acheive SVR.

    As far as the recent reports around HCCs and Tx goes, they are very preliminary at the moment and may or may not point to some concerns. But for us I believe that it’s too late to be crying over any potentially spilt milk so to speak and we should just take it as an advisory to keep a careful eye and regular check with scans and tests. I would recommend to keep to your MRI schedule rather than delaying and while AFP is only a rough indicator, it is a quick, cheap and mostly painless way to monitor for those gradual increases or sudden jumps that may indicate a need for a closer look.

    The UK’s laws on import of prescription medications are quite liberal, the main issue seems to be getting UK doctors to write scripts. Your GPs script will be fine and I assume would be based your hepatologist’s advice as to type and length of treatment?

    Otherwise a script from an online consultation with http://www.gp2u.com.au or your Australian based doctor would also work. In those cases you would need to supply your medical history for HCV including genotype and liver fibrosis stage, etc.

    That your Hepatologist is prepared to monitor is great news.

    We do need to be careful with what we read into this recent report. It is really an advisory of initial findings that may not be totally cohort neutral. Due to the retrospective nature of the study there may have been some selection in favour of those with HCC recurrence which is why additional study has commenced.

    Also we should keep in mind something one of the study authors points out:

    “This is a different cancer than elsewhere in oncology — it is a cancer within an advanced chronic disease — so the prognosis, the life expectancy, is related not only to the liver cancer but also to the liver disease and liver function,” he explained. “If you don’t treat these patients and ameliorate their liver function, and if hepatocellular carcinoma occurs, you have no chance of curing them. But if you ameliorate liver function and they develop hepatocellular carcinoma, you can cure it better because their improved liver function will allow an ablation.”

    So for those with prior HCC who are now treating with DAAs it should act as an advisory to instigate careful and more regular than usual monitoring during the initial period post EOT.

    Edit: yes Jeff you are correct about share prices, always look at the authors declarations of interest too.
