    Hi Paul,

No apparently the nurse can’t even determine that. Not a lack of experience either so I’m keen to hear what she finds out next week.

Errrrm, yeah! Who knows? (we really need an animated shrug) The frustrating bit is that with Xmas/New Year and people on leave, etc. I didn’t get that result for over three weeks, unable to determine what went wrong and now waiting on retest results.

Basic qualitative, no attempt to quantify even whether above or below limit of detection, just “RNA Present: Yes/No”.

The answer was Yes so I have HCV apparently

    Hi Sir,

Thanks for that post, very informative. I agree with you about running fewer tests under new DAAs however in my case (and from what Jaz said also her) the lab only ran the basic qualitative test so all my clinic could determine was that I have HCV. Now this test would have been fine if I had come up as undetected but I didn’t so currently they don’t know if my V/l has gone down, up or even sideways, which is why they have requested the lab to retest. In theory my LFT dropping may indicate response but it has dropped to those levels in the past although not for a long time. I’m not particularly worried or feeling rushed given the need to treat for 24wks anyway but with prior exposure to Dac in a trial 3 years ago it would be nice to confirm no ns5a resistance by actual results sooner rather than later. :)

    Hi pkhow,

Your last choice! The results are so close anyway that until millions are treated we won’t know with any certainty. And then it will only be statistics from previous history and not what will happen next time.

Anyway you were UND at 4 weeks so looks like you made the right choice, relax and enjoy. You are well on the way to SVR. :cheer:

Thanks Tina, I should also have commented that what concerns me more than these open declarations is when you have to dig down into the nitty gritty of annual financial reports to look for funds being provided.

    Hey LG
Bang on – if I may draw your attention to Appendix 6: Summary of declared interests of members of Guidelines Development Group for the development of WHO HCV guidelines for screening care and treatment of persons with HCV.

Makes for some interesting reading and shows the extent of the greedy paws of big pharma

    Thanks Tina,

It is appropriate that this is posted but I also think that we need to be careful about accusations of attempts to unduly influence by the Pharma’s and being unduly influenced against the guidelines creators.

Part of this issue is that the various government approval processes such as PBAC and FDA require approval trials to be monitored by independent monitors. Usually this will be large hospitals that will not have the funds to divert from their normal budgets and governments are not prepared to cough up the money to support a commercial enterprise. So the Pharma’s provide the drugs and funding for the trials. Other times they provide funds for experts to travel, meet, cooperate internationally for the benefit of many (err….including themselves). In an ideal world it would all be arms length with truly independent arbitrators in the middle. The real world doesn’t work like that so we always have potential for conflict of interest. The intent of these lists is to make us aware of where that potential lies so we can watch for anything untoward. Really a case of “Better the devil you know….”

My declaration of interests: Drugs, health monitoring, parking and pro rata travel costs funded by BMS as participant in clinical trial. :P

On the subject of vitamins, I would note that most people are unlikely to develop serious, debilitating illness or die if the vitamin manufacturers price their product at levels that exclude all except the wealthiest individuals. Therefore the choice to purchase their products or not ultimately comes down to personal choice and belief systems.

Thanks Greedfighter – you know why B)

I just wonder whether ‘resetting’ your body clock is going to make a difference here. I suspect it is not the body clock being out alignment with the rest of the planet like with shift work or travel but the meds just making it difficult to stay switched off. From what I’m experiencing and reading from others, it doesn’t appear to be so much of a difficulty going to sleep rather more with staying asleep?

The point being made by the doctor I mentioned at the bottom of my post was that there was no “best” as all methods carry some risk and all (except the first) have varying levels of accuracy. Your decision needs to be based on how important is it to be totally accurate compared with the risk. The decision process for HCV does not require total accuracy to make decisions. If your fibrosis level is low by any of those methods then short treament should be fine, if it is high then long treatment. If it is close to requiring long treatment or varies wildly from a previous one then either retest, use another method or consider treating for longer.
