    Totally agree, my remark was not intended to claim that 10% is a nice number…. It is not at all. Australia is in any case leading the path for people to get treated for HCV and has made a lot of progress by compare with other countries. Whether it could be better, oh yes, they can be better, but aren’t… So I am with you on this.

    My remark was in fact a sad ascertainment towards the situation from Romania and thinking that if at least 10% would have been treated in Romania that would still be something by compare with the currrent status quo. Without going too much in details (as I did already in others posts) – people in RO get a lot of promises about the new treatment, but when trying to get treated, apart from the expensive treatment there exist also bureaucratic barriers (like you need to get a voucher from your doctor to make the blood tests and fibromax to a certain laboratory, yet the doctor does not receive these vouchets or receives only couple of them… And during this time he/she has hundreds of patients waiting to be treated).
    What you in Australia have now seems to be (actually it is) mission impossible in Romania.
    And to make things even worse, there are many people ( in RO) who think about the generic HCV drugs as being a kind of, let’s say it nicely, “occult” thing….
    Some weeks ago I posted in another thread that the president of the NHS from Romania (CNAS) made a statement that from the Fall of 2016 other HCV patients, apart F4 stages, might be treated, but the reality is that they try to include F3 patients into treatment, but not the F0-F2 patients (there was an article published somewhere, but I cannot remember where). This means that for people like me, if there would have been no Australia, no fixhepc, no Greg Jefferys, I would have been sick and on the way to F3, then F4, then …..

    10% seems a small percentage, but when I think that last year in Romania less than 1% from the HCV patients received the new interferon-free treatment, 10% looks like a huge number…..

    But, the news about DAA generics is already spreading, so I am pretty sure the number of people being treated will increase.

    here is a link about the studies being made for SOF+VEL when treating for GT3 and GT2 patients. To be noted that GT3 success rate is increased, at 95% (ASTRAL-3 Trial).

    Hi Everybody,

    couple of feedbacks regarding my previous post:

    & Matt: I posted the link about Greg’s post in order to highlight the aspect about the anxiety felt by patients when they reach EOT and the waiting time of 12 weeks. It provided also couple of arguments about possible causes of relapse. My intention (and probably I should have said this from the beginning) was to touch the idea of having trust in the DAAs despite the anxiety during the waiting times.

    My understanding regarding the mechanism of action was that Chlorcyclizine might block HCV in entering the liver cells. So it is a different mechanism than the DAAs which affects the replication of HCV. On one side, the idea of using it sounds like an additional protection, after the DAAs treatment is finished. On the other side, I have in mind that outside the body, in dried blood, HCV may survive upto 6 weeks, while inside the body there is a more “friendly environment” for the HCV, therefore if after the DAAs treatment there will still be some RAVs hidden somewhere in the body, the Chlorcyclizine might need to be used for more weeks.
    Who knows, maybe DAAs + Chlorcyclizine might be something like the case of getting rid of the bugs: you put poison (aka you take the DAAs) and the bugs getting in contact with it will die, but some bugs will remain in a crack in wall and might survive, so if you block that crack, the bugs will finally die from starvation :D. It would be a nice dream. Yet dreams are dreams, reality is reality and sometimes dreams do become reality ;-), so it is hard (at least for me at this moment) to give solid opinions.

    Anyway, please consider my “message of caution” not as a kind of “do not do it”, rather as a kind of “research more, ask more, get a professional view and if it turns to be (or might be) safe for you, give it a try”.

    Hi Fitz,

    an interesting information, which is new to me.
    However, I just read the article from the first link you posted and I became …. cautious with regard to the remark that this substance may cross the blood-brain barrier ….

    As you are approaching EOT, it might be that your level of anxiety increases…..It happened to others too :), so just think it is normal to feel like this and enjoy the time till week 12 after EOT doing something else, maybe not related to HCV ;-).

    The great thing in the present times is that the information spreads much much faster via www.

    Regarding the prices of the products, as long as the products are dedicated / intended to be sold in mass, the price will decrease in time in order to reach the large scale levels of distribution. Take whatever example you want where the product contained a high level of innovation (cars, computer/notebooks) – the initial price was very high, few were able to afford it, but decreased over time in order to reach large scale of distribution. The so called IP rights which should guarantee a high degree of innovation turns unfortunately today to be a mechanism to make huge amounts of money and slowing down the large scale distribution….Things may not and for sure will not remain like this. No matter how much greed might exist, there will come one day the moment of time when the governments will see that the IP laws need to be revised and the “windows” by which Big Pharma makes money will become smaller.

    Gilead just published the Q2/2016 sales results.

    Couple of highlights (usually Q2 is the best quarter for sales):

    – Product Sales
    Total product sales for the second quarter of 2016 were $7.7 billion compared to $8.1 billion for the same period in 2015. Product sales for the second quarter of 2016 were $4.9 billion in the U.S., $1.6 billion in Europe, $619 million in Japan and $531 million in other locations. Product sales for the second quarter of 2015 were $5.6 billion in the U.S., $2.0 billion in Europe, $62 million in Japan and $515 million in other locations.
    – HCV product sales, which consist of Harvoni® (ledipasvir 90 mg/sofosbuvir 400 mg), Sovaldi® (sofosbuvir 400 mg) and Epclusa® (sofosbuvir 400 mg/velpatasvir 100 mg), were $4.0 billion compared to $4.9 billion for the same period in 2015 primarily due to a decline in sales of Harvoni.

    I think there is not much to comnent, apart from the fact that with a market of 150 000 000 patients worldwide, the HCV sales are dropping…. A basic rule of profit says that you can make nice profit going to mass distribution and lowering the price.
    P.s. How many zeros are in the exercise:
    150 000 000 x 1 000?
    If you answer “more than 20 zeros” you have high chances to get hired in a profitable pharma company :d (this was a poor attempt to make a joke … )

    Hi Fitz,

    …thank you for sharing it. It made me reflect again that HCV must be treated asap, no matter on which stage the liver is.

    And, if allowed, another thought: I do not get the picture why people talk so often/much about the 500 000 people that die yearly because of HCV and neglect the rest of 150 000 000 infected people who might live a better life if they get treated sooner. It is not fair, because it creates the impression that HCV is in the end not such a big issue. My brain cannot imagine how big a crowd of 150 000 000 people look like…

    First few days after EOT I felt this discomfort (moderate) as well.
    After receiving the EOT blood test result, the bilirubin level was slightly higher than the max normal value. However, after the UND result was again confirmed at EOT, I got relaxed and the discomfort diminished. Now, 1 week past EOT, no more discomfort.

    Somehow I tend to believe that the stress (or anxiety of no longer taking the HCV drugs) had a certain influence. One other factor that I believe might have lead to the discomfort was the fact that in the last 4 weeks of treatment I was eating more and gained some weight.

    Hi Fitz,

    don’t be upset on your GI for not having written you the VL blood test.
    In Romania I had to pay all my blood tests on my own. When going on the path with generics, everybody tells you it “is your own decision” (that includes also the risks and costs).
    Because the VL blood test is so expensive, as far as I know my doctor would be held accountable if he would have written me a VL analysis. I posted in another thread here on this forum, that in Romania you need to get a kind of voucher to make such analysis and you are making it only if you are on the point to start the treatment. And as long as I was not F4, it does not make sense to “waste” a voucher on me….. There exist a kind of “money police” and if doctor does prescribe certain blood tests in a context outside the regulated one, the doctor will be …… punished :angry: as he/she is wasting the taxpayer’s money….

    In a certain extent, I do understand the stupidity of the context: you get treated on your own money, get cured, save +100K $ from the national budget, yet the “System” tells you that as long as the treatment with generics is not approved by FDA / NHS / (or whatever other authority), you are not entitled to refund neither to be supported with blood analysis. So…. I think we all here are a kind of vikings….Brave and alone on our journey over the seas.

    About your cholesterol level, any stress lately? As far as I know, there exist a link between stress level and cholesterol.
