    Well, I always like a good debate, and I can accept other peoples point of view, even if I don’t always agree. In the end, if I don’t agree with someone, its because I think I understand the basic facts.

    Concerning today’s treatments for Hepatitis C, the basic facts are that the price charged by Gilead for a life-saving treatment are extortionately high for those that can afford it, and absolutely out of reach for those who cannot. The end result is that hundreds of thousands of people are dying each year and millions more are suffering needlessly. So please don’t anyone try to tell me its a complex problem. Its only the solution that seems complex… But only if you want it to be.

    Dear A.L.,

    If you bought a new top of the range car with all the latest gadgets (speed control, air conditioning, parking assist, econo-boost engine, interior LEDs, the lot) for $50 000, you’d probably be pretty happy with the deal, right?

    Suppose you then learn that the true cost of manufacturing that car (including all R&D and a fair profit margin) was only $1 000?

    How would you feel about that?

    In my opinion, Fibroscan only gives a rough indication of the F-score… I had one done, and I’m sure if a different nurse did the test or if it was done on a different day then the Kpa number would probably have been a bit different. But compared to having a biopsy (sample of liver extracted with a terrifying-looking needle) it is non-invasive.

    Little personal story:

    I also had a liver biopsy done. My specialist told be that he normally does it under a general anaesthetic (for the patient not for him ;) ), because apparently it can sometimes be painful. But because I didn’t know better, I turned up at the clinic having had a full breakfast and he refused to give me a general anaesthetic. So he gave me a local anaesthetic instead, and then did the biopsy while I watched (no pain at all). Weird experience!

    In the end, the biopsy showed F0, but the Fibroscan showed F0/F1. Which one was more accurate? Most probably the biopsy. But maybe the sample came from a still-healthy bit of the liver? Or more likely, the fact that I also have a couple of harmless cysts in the liver threw off the fibroscan result a bit. The cysts are just filled with fluid, so they will have a different density from the rest of the liver…

    Probably everyone on the forum will agree that the F-score should only be used as a rough guide for how many weeks of treatment should be followed, and not as a justification for ware-housing people who are infected with a seriously dangerous virus….

    1.97E+4 = 1.97 x 10^4 = 1.97 x 10000 = 19700.

    This is low if is before treatment. Still a bit high if you have started treatment.

    I totally agree with abal’s advice that FixHepC should not be encouraging people to break the law in his country. FixHepC must remain whiter than white in all countries. FixHepC must stay legal to be able to keep the moral high ground.

But show the evidence! Me, I don’t think anyone needs to show any evidence that buying prescription drugs in India without a prescription is *illegal*. But I would be very interested to see hard evidence to show that is in fact legal.

    I have said it once before, and I hope I don’t have to say it again. There are people on this site who shoot before they think…

    To continue my rant about NHS Good-Speak, here is a good example:

    A recent news article on the BBC describes how Hepatitis C came to Scotland “through the mass treatment of soldiers in field hospitals across the country during WWII. …”

    OK, fair enough. Hep-C goes back to WW2. But then the article goes on:

    Within Scotland, transmission of the virus began to increase in the 1970s, with different strains being discovered in Glasgow and Edinburgh. … From then until 1991, about 2,500 people were infected with hepatitis C through NHS blood products – mainly those imported from the US – during a contaminated blood scandal described by a Scottish inquiry as “the stuff of nightmares”. Children being treated at Yorkhill Hospital in Glasgow for haemophilia during this time were given plasma products sourced from donors in the US, which were known to be high-risk. As a result, many were infected with HIV and Hepatitis C by the age of five. Lord Penrose launched a six-year inquiry, the conclusions of which were met with cries of ‘whitewash’ from angry victims and their families when it was published in March 2015.

    So much for progress. Maybe we can learn something from all that… But the thing that really bothers me is how the authors of the latest study now want to leap into action:

    “Lead scientist of the University of Glasgow study, Dr Carol McWilliam Leitch, said: “HCV poses a significant public health challenge in Scotland as well as globally. There is currently no vaccine against the virus and the recently developed antiviral drugs are not only extremely costly, but resistant strains have already emerged. Pinpointing regions of Scotland driving HCV spread will allow us to more effectively target treatments, monitor their effect and track resistant strains. These measures are essential if we are to combat the virus. We now intend to focus our attention on HCV spread in other Scottish regions and to extend the study across the UK.”

    OK, Dr Carol McWilliam Leitch works for the University of Glasgow, and not the NHS. But I ask you!!??

    I ask you again! Why is there no mention of actually treating anyone in this article??

    Tell me about it! Along with all those enormously expensive conferences and meetings that a large variety of teams, charities, medical people, think-tanks, etc etc and business types hold and then make some sort of statement that means absolutely nothing to us, the patients.

    Right. I just spent the last two hours watching the presentations of those guys at the Paris Hepatitis Conference. They are all obviously well-meaning people. and they have devoted their careers to trying to treat liver disease. And no doubt well-paid careers, I must say (did you see the number of suits in the audience, and the number of declarations of interest in the slides?).

    And all those good Drs are all fully aware of cost of Gilead’s treatment. But nobody seems ready to really talk about it head-on. It is as if COST is a four-letter word to those guys, even the most well-meaning. The brave ones mentioned the C-word a few times, but only in passing between slides. Hey! I said the C-word once but I think I got away with it….

    … Don’t laugh, you stupid patients, we’re trying to help you! Will you please stop mentioning the COST!
