Attached is the toxic substance control act form customs will have you complete if they hold your drugs. Is there a correct way to complete this form or can you choose either option and still get your drugs. Hopefully the form shows up.

G1a dx’d in 1992, Biopsy F2 VL 8mill +. Tried tx with Interferon/Riba, back in 2008 didn’t last long it felt horribly ugly!! I stopped tx, after 5 weeks!!
Started tx 6/1/16 with Harvoni.

Wow! Surprised I haven’t rec’d an answer for this yet. I ask b/c I got a bill from FedEx for 89 cents. Is that common?

G1a dx’d in 1992, Biopsy F2 VL 8mill +. Tried tx with Interferon/Riba, back in 2008 didn’t last long it felt horribly ugly!! I stopped tx, after 5 weeks!!

Is the first one a trick question? I’m not even going to try to second guess US customs!

From memory FedEx can charge you for any customs charges they incur on your behalf, but 89 cents?!?!

I’ve never seen anyone say they got something like that. But I have dealt with government agencies extensively in the past.

I hope you plan on paying that!

Absolutely, I’ll pay it Monday after my appointment downtown, that way I’ll be near a FedEx facility. Incidentally the right choice is a Negative Certification. I think you’ll get your drugs either way and you only have to fill it out if Customs contacts you and asks you to fill it out. So said, the top choice may bring more questions, anyone know? Me thinks not, cause nobody answered in the first place.
