    I’m I U.K. And just been to Hospital and they have agreed to monitor me if I purchase online medication. They have taken blood tests and am awaiting results so that I can purchase. I’m genotype 1a treatment naive with viral load of 1.6m and they have said I wld only need 8 weeks of Harvoni. Your redemption trials indicate 12 weeks. Do I have to buy this amount or will GP2U only prescribe the 8 weeks?

    Hi Mic,

    It is true that some patients could can clear within 8 weeks, but I for one was not one of those and I and so glad I treated through MonkMed /FixHepC . If under the NHS advise, I would have definitely relapsed quite early on in the game.

    The recommended treatment is 12 weeks for treatment naive, The NHS 8 week recommendation is based on cost-cutting, nothing else.

    I’m sure Dr Freeman (Fix HepC ) or Dr Debasis (MonkMed) would advise you the same.

    My feeling is those who have carried the virus many years in particular, should do the full 12 weeks, I ended up needing 24 weeks of medicine, although I was one of a very small % who went in the slow lane .

    I highly recommend also doing a 2nd/3rd viral load test at 8 weeks if the NHS only offer one at 2 and/or 4 weeks into treatment. Even if you ask your GP for it or pay privately (MediChecks a reasonable price to others).

    Advise don’t do what the NHS suggest without seeking a 2nd opinion by the Drs mentioned above who are not restricted by bureaucracy and cost-cuttiing budgets at patients’ expense.

    8 weeks should do. 12 weeks can be viewed as 4 weeks insurance. Either is reasonable and both available.

    Thanks for your quick response. So would you advise me to buy the full 12 weeks just in case I haven’t cleared the virus at 8?

    The guidelines, based on the Gilead trials suggest 8 weeks as sufficient to get the same results as 12 weeks in treatment naive, low fibrosis patients with a VL of less than 6 million

    BUT

    According to Gilead the SVR rate for 12 weeks Harvoni in treatment naive patients is 97%

    HOWEVER

    According to the VA in the states, which gave it to over 4000 veterans (10x what Gilead did) and then reported the results the real world success rate is 91.3% – ie the failure rate is 3 x higher than suggested. This raises the issue of how much we should rely on the 8 week is ok data. Here is a link to that study:

    So, if it was me I would be considering taking 12 weeks unless finances were such that the cost of the extra 4 was prohibitive.

    One useful thing the VA study did show is that undetected at 4 weeks is a good sign and that detected at 4 weeks reduces SVR rates by 8-10%. This is part of the emerging evidence that response on treatment does have some guidance value.

    Hi Mic,
    We are (were) almost identical from an HCV standpoint. (v/l, gt, etc)
    My doc in the US also recommended 8 weeks, but I went with 12 to be absolutely damn sure I had done all that I reasonably could to get rid of this crap.
    It worked.
    If you were paying $30,000 for 4 weeks, it might be something worth considering.
    If the side effects were unmanageable, the same.
    But that is not the case here.
    4 weeks to destroy virtually all the virus.
    4 weeks to mop up any stragglers.
    4 weeks to make damn sure you got every last one.
