Essentially you don’t strictly require any monitoring unless you get unwell but:
4/54 == 4 weeks post starting treatment
FBC = Full Blood Count = CBC = Complete Blood Count
Cr&E = Creatinine & Electrolytes = UEC = Urea Electrolytes Creatinine
LFTs = Liver Function Tests like Bilirubin, ALP, AST, GGT, Albumin
Hep C Viral Load = Quantitative HCV RNAThe 4 week test typically shows (80%) viral load zero and proves the meds are valid and working.
If you want an early proof your AST will fall rapidly back to normal
Once viral load is zero there is really not a lot of need for further monitoring (unless on Ribavirin)
The 12 weeks versus 24 weeks length decision is based on Fibrosis. If you have cirrhosis (F4, > 14 kPa, Hepascore > 0.5) then you need 24 weeks.
Next test after viral load zero can be SVR4 or SVR12 – Viral load 4 or 12 weeks post finishing treatment, and a VL=0 at 4 weeks = 96% chance of cure, and at 12 weeks 99% chance.

It will be the Riba making you feel bad. If you read http://fixhepc.com/forum/gt3/369-gt3-high-svr-rates-with-daclatasvir.html you will find an n=468 study (the biggest Sof+Dac study to date) that failed to show any benefit in an subgroup of F3 & F4 GT3 patients by taking Ribavirin. If you are doing 24 weeks this suggests that your results will be equal or better – Riba than + Riba.
Ribavirin has a 1/2 life of 12 days so when you stop it takes 12 days to fall to 1/2 the starting level, 24 days to fall to 1/4, 36 days to fall to 1/8.
I understand why intuitively it might seem “more is better” (and therefore + Riba is better) but the evidence suggests that is not the case.

Glad you had a lovely Xmas with your family. I finish mid February as well however only 12 weeks for me. I did read where you were going to take an extra 6 weeks…is this something new or more a personal choice? Just curious as I am 1b as well.

I have acquired a taste for Coopers ultra light beer which had 0.05% alcohol (don’t drink it a lot though) and also Holstens non-alcoholic beer….sounds gross but isn’t a bad drop once you get used to it. I miss my Bacardi though

Absolutely correct. It is possible to get to purity values of 99, 99.9, 99.99, 99.999 but each extra “9” you want increases the price by a factor of about 9!
HPLC can deliver near 100% purity but does not work at scale, so other more time/cost efficient methods of purification are used.
With residual solvents near enough is good enough. They are normal, expected, and will have no impact on efficacy.

Hi Joan When you say Viral load 0…did that actually show “0” on your test or did it just read “undetected”…I admit I am confused on that point…although my test at four week was HCV undetected so assume that means no Hep C in my system anymore.
So glad you are doing so well. My sleep is still not so good, waking early all the time but generally I feel so much better.

You should see a doctor about the ankle swelling.
I have a handful of patients who have developed fluid retention on treatment initiation.
It can be treated with diuretics but should not be ignored. It can progress to ascites.
Please check in with a doctor.

The < 15 and < log 1.18 (1.2) are not printed by many labs when it is undetected. We do see <15 Detected Which means too low to count but still present. Yours is too low to count AND can't see any. It is zero.

There are several different ways results are reported.
If a QUALITATIVE HCV RNA is requested the what you get back is a simple DETECTED/UNDETECTED
If a QUANTITATIVE HVC RNA is requested then the results may look like
< 15 Detected <15 Undetected N (where the <15 would go) N=HCV RNA not detected Where there is a <15 the is also often a < log 1.2 reported. If you post the data bits of your results I will tell you what it means. If it says undetected somewhere that means we can't see it (as well as it being <15)
