    I agree to respectively disagree with the likes of Roy M Poses about his extreme views on EBM.

    There are always extremist views within any given profession and a tsunami of data on any given subject.

    The trick is to separate the wheat from the chaff.

    I wish you the best in deciding on the appropriate HCV treatment for your circumstances based on an acceptable degree of confidence according to your criteria.

    Hi Serg

    Thanks for your verbose and entertaining posts in which you argue yourself into a loose granny knot about the need for long-term EBM over “ … many years of observation (or even decades for people with mild fibrosis)” before HCV DAAs can be an ethically approved treatment according to your strict interpretation of EBM.

    Gaj has easily untied your loose granny knot arguments and exposed then for what they are. There’s nothing further that needs to be added except for a truism attributed to John Maynard Keynes:

    “In the long-run, we’re all dead.”

    Thank god that what you espouse is not followed, as most of us on this forum would not be alive in the “decades” time, that are needed according to your strict EBM criteria, to ethically approve the currently FDA approved HCV DAAs.

    The only medically unethical thing about the currently FDA approved HCV DAAs is the abhorrent abuse of patent laws used by big pharma to charge exorbitant monopoly prices for them. You are better advised to take up your arguments with the patent holders.

    Welcome aboard the FixHepC longship. I wish you smooth sailing in your quest to slay the HCV dragon. You are well equipped to do so with your generic DAAs as your weapon of choice, and with plenty of battle hardened FixHepC warriors behind you willing to offer you back-up support should you need it. Draw on your presumed Scandinavian heritage by evoking Eric the Red’s fighting spirit, and use the symbolic dragon head on his longship’s bow for good luck, to slay the HCV dragon.

    Hi Gaj

    I’m sorry to read that the virus is detected again. The GT3a virus seems to have mutated into the boomerang genotype, as it keeps on coming back. However, SOF + VEL looks like a promising weapon to eliminate it. The most problematic NS5A RAV for this combo appears to be the Y93H variant, so hopefully your baseline tests will not find it.

    Stay positive and patient. I have no doubt that you’ll eventually beat it with persistence and determination.

    I love the photo of Ruby and James.

    Yes, I was lucky to be undetected at 4 weeks, which is pretty common for GT2s on generic SOF + DCV from my understanding. The real test is in a few weeks’ time when I get tested for SVR4.

    I’m not worried about it, however, as I’m feeling fine at the moment and there are other options available should I relapse.

    I simply try to keep a positive attitude, without being complacent, or deluded like the Black Knight in Monty Python and the Holy Grail who after having all four limbs severed thinks that it’s just a flesh wound.

    It’s also therapeutic to have a laugh at ourselves and the world as we see it. It’s no good whinging and complaining all the time as it falls on deaf ears.

    It sounds like you are in good spirits now that you are well and truly cured.

    I’m glad to hear that you have started your treatment. Thanks for honestly sharing your decision process.

    I remember how sceptical you were … coming on the forum frequently asking a lot of questions … it must have seemed too good to be true to you … strangers on the other side of the world offering to deliver relatively inexpensive HCV DAAs to your door.

    I hope that the abstract in the Journal of Hepatology, of Dr Freeman’s 2016 EASL presentation, that I posted helped to convince you that this is a legitimate HCV forum with bona fide generic cure paths.

    Conclusions
    Baseline RASs in NS5A have minimal effects on patients’ response to ledipasvir/sofosbuvir therapy. When these RASs do have effects, they could be largely overcome by extending treatment duration or through treatment intensification.
